Prior to soliciting an application, please check to see if you are contracted with that carrier and appointed in the correct state. You can view this information and submit all contracting and state appointment requests electronically through our automated system, SuranceBay. When processing a new application, please have your client complete our general HIPAA Authorization Form. This will allow us to shop the case with multiple carriers for the most competitive offer.
